Faculty notification request for emergencies
The Dean of Students Office can assist you by verifying the absence was due to an emergency or critical situation and notifying your instructor. Note: our office does not excuse student absences. It is at the discretion of your instructor whether to excuse an absence.
Examples of emergency and critical situations:
- Death of a family member
- Emergency hospitalization
- Apartment fire or natural disaster
You may need to provide supporting documentation:
- Death certificates
- Obituaries/programs with name of deceased displayed
- Medical discharge documents
- Reports from first responders
